Generally, patients from the Priority Care program are transferred to the Palliative Care program toward the final stages of their disease. The Palliative Care program is provided to Sierra Medical Group members and is part of the home health benefit. Patients generally have a deteriorating medical condition and are likely to need assistance with symptom management. Patients may have an average life expectancy of a year or less, and are generally homebound or unable to easily leave home for medical care. Pain and symptoms control related to physical, emotional, and spiritual concerns are evaluated and managed. Our goal is to achieve comfort, improve quality of life, and provide support for patients and caregivers.
