Work In Partnership With Your Doctor to Prevent Medical Errors

Make sure that all of your doctors know about all of the medications you are taking.  This includes prescription and over-the-counter medicines as well as dietary supplements such as vitamins and herbs.  Many prescription medications and herbal remedies can have serious side affects when used together.  At least once a year, put all of your medicines and supplements into a bag and bring them along to your doctor appointment.  This will help your doctor keep your records up to date, which can help you get better quality care.

Make sure your doctors know about any allergies or adverse reactions you have had to medications in the past.  Your doctor can be certain to avoid this medication or another similar one that might harm you.

Ask about your medication in terms you can understand.  Do this with your doctor and again with the pharmacist.

  • What is the medicine for?
  • How am I supposed to take it and for how long?
  • What side effects are likely?  What should I do if they occur?
  • Is this medicine safe to take with the other medicines/dietary
    supplements I am taking?

If you have any questions about the directions on the medicine label, ask your doctor or pharmacist to answer them.  Instructions can be hard to understand.  For example, if you are supposed to take “four times daily” does this mean a dose every 6 hours around the clock or just during regular waking hours.

If you are scheduled for surgery, don’t forget to remind your doctor about all of your regular medications.  Many herbal remedies may cause increased bleeding and should be stopped at least three weeks prior to surgery, if possible.  Many patients misunderstand the instructions to not eat or drink anything before surgery as do not take your medicine.  Bring all of your medication with you to the hospital or surgery center to make sure the doctor knows what you are taking.
